Acute microvascular changes may be associated with the development of organ dysfunction in critically ill patients. Propofol significantly reduces vascular tone and venous return, which can result in low arterial pressure. However, the effect of propofol on normal human microcirculation is not known at present. Koch M and colleagues at the University of Brussels in Belgium conducted a prospective, open-label trial in 15 patients who received propofol anesthesia for vaginal oocytes. Researchers used orthogonal polarization spectroscopy to study the sublingual microcirculation network before and after infusion of propofol and during infusion. The results showed that the average (sd) calculation of the propofol effect site concentration was 6.5 (1.8) μg/ml. There was no change in systemic hemodynamics and oxygenation variables during propofol use, but overall microvessel density decreased by 9.1% (P < 0.05). There was no change in venule density, but the density of perfused capillaries was significantly reduced by 16.7% (P<0.05). Three hours after stopping the propofol infusion, the change in the microcirculation disappeared. Koch et al concluded that propofol infusion anesthesia can reduce capillary blood flow.
