On the 23rd, Taiyuan City released the "Ishi (Taiyuan City) Deepening Medical and Health System Reform Implementation Plan," outlining a series of new policies across five key areas: medical security systems, basic drug systems, medical service systems, health service equity, and public hospital reforms. **Highlight 1: Increased Medical Insurance Payment Limits**
Currently, urban workers' medical insurance, urban residents' medical insurance, and the new rural cooperative medical system cover all urban and rural residents. The participation rate for these three basic insurance schemes has exceeded 70%, with an expected rise to 80% in 2010 and over 90% in the following three years.
Within two years, all retired employees from bankrupt enterprises and workers from struggling companies will be included in the urban workers’ medical insurance. Those facing difficulties can apply to join the urban residents’ insurance. College students will also be covered under the urban residents’ insurance. These efforts are already underway and are expected to be completed this year.
Employees in non-public enterprises can choose flexible employment options to join either the urban workers’ or urban residents’ insurance. Migrant workers who struggle to enroll in the urban workers’ insurance can opt for the urban residents’ insurance or the new rural cooperative system in their registered area.
Starting in 2010, the maximum payment limit for the three basic insurances will increase from twice the average annual wage of employees, disposable income of urban residents, and per capita net income of farmers to about six times. The reimbursement rates for hospitalization and major outpatient expenses will also rise. For insured urban employees, hospitalization expenses will be reimbursed at 70%, while urban residents will see 50%. In 2010, farmers participating in the city’s insurance will have hospitalization costs directly reported at the hospital, and by 2011, they will be able to settle bills province-wide.
**Highlight 2: Zero-Rate Sales of National Essential Medicines**
The city will implement the national essential medicine system, establishing a mechanism for selecting and adjusting local essential medicines. Government-run medical institutions will focus on procurement, distribution, and zero-rate sales at purchase prices.
Strict regulations will govern the use of drugs in medical institutions, and essential medicines will be closely monitored. By 2011, the sampling rate for nationally listed essential drugs will reach 100%. Starting this year, all retail pharmacies and medical institutions must stock and sell national essential drugs. All essential medicines will be included in the basic medical insurance reimbursement list, with significantly higher reimbursement rates than non-essential drugs.
**Highlight 3: Village Coverage, Township Standards, County Improvement**
Within two years, the goal of “village coverage, township standards, and county improvement†will be achieved. This means each village will have a health center, township health centers will meet national standards, and county hospitals will enhance their service quality.
From 2009 to 2011, the city will focus on strengthening four county-level hospitals, with two completed this year. Twelve central township health centers will be built, and 11 will be completed this year. This year, 40 to 60 square meter buildings will be constructed for 126 administrative villages without existing clinic facilities. Village clinic coverage will reach 100% this year, with each clinic equipped with at least 5,000 yuan worth of equipment.
Over the past three years, 51 community health centers and 225 community health stations have been built or renovated, ensuring that every administrative village has at least one qualified village doctor.
